InterWell Health is further expanding its kidney care services with the addition of Acumen Physician Solutions.
InterWell will acquire Acumen, which provides electronic health record and patient management software for nephrologists. InterWell was formed from the three-way merger of Cricket Health, Fresenius Health Partners and the original InterWell.
Acumen was formerly a division of Fresenius Medical Care North America.
“The addition of Acumen accelerates our ability to provide the most comprehensive offering to kidney care providers to support their move to value-based care,” said Robert Sepucha, CEO of InterWell Health, in the announcement.

“This technology is a perfect complement to our existing tools that are identifying high risk patients to reduce hospitalizations, and connecting patients with leading education platforms, care teams, and peer mentors,” Sepucha added. “We are committed to slowing the progression of disease, increasing transplant rates, accelerating adoption of home dialysis, and improving health equity.”
Acumen’s Epic Connect platform has been deployed by more than 2,000 kidney care providers and was designed in partnership with Epic, one of the largest EHR companies in the country. The platform is designed to support value-based care and includes payer integration, a physician rounding tool, embedded analytics, population health management options and interoperability with other providers.
Most (65%) of InterWell’s network has adopted with the Epic Connect platform, the companies said.

The InterWell-Acumen deal does not require regulatory review, according to the announcement.
